# German-PKV.de > German-PKV.de helps international professionals, freelancers and researchers in Germany decide between public (gesetzliche Krankenversicherung) and private health insurance (private Krankenversicherung). It explains the income threshold for 2026 (€77,400) and 2027 (€84,150, draft), contributions, the employer contribution, family insurance and what happens to a policy when you leave Germany – honestly, including when public insurance is the better choice. German-PKV.de is a free information and calculator portal and not an insurance broker; on request, enquiries are passed on to specialised experts from a partner network (insurance brokers licensed under § 34d (1) GewO). Not immigration or legal advice. As of October 2026. ## Key facts (with sources) - Employees in Germany may choose private health insurance only if their regular gross annual salary exceeds the income threshold: €77,400 in 2026 and €84,150 in 2027 (Federal Cabinet decision of 7 October 2026, Bundesrat approval pending; § 6 SGB V). - Employees who are already privately insured and exempt on 31 December 2026 are subject to a lower 2027 threshold of €80,550 (§ 6 (8) SGB V as amended, draft value). - If a salary rises above the threshold in an existing job, compulsory public insurance ends at the end of the calendar year – but only if the salary also exceeds the next year's threshold (§ 6 (4) SGB V). - Anyone who takes up their first job in Germany with a salary above the threshold can join public health insurance voluntarily within three months, without prior insurance periods (§ 9 (1) no. 3 SGB V). - The minimum gross salary for an EU Blue Card in 2026 is €50,700, or €45,934.20 in shortage occupations (Federal Ministry of the Interior, Federal Gazette of 18 December 2025) – well below the health insurance threshold. - In 2027 an employee earning above the contribution ceiling pays up to €729.94 a month (childless) or €672.56 (with a child) for public health and long-term care insurance (own calculation: 2.9% additional contribution, draft care reform). - Employers pay half of a private health insurance premium, up to €613.22 a month in 2026 and about €672.56 in 2027 (scenario); the health part (€508.59 / €557.81) and the long-term care part (€104.63 / €114.75) are capped separately (§ 257 SGB V, § 61 SGB XI). - Stiftung Warentest found full-cover private tariffs for 35-year-old employees and self-employed people costing €649 to €1,137 a month; only 384 of 1,245 tariff combinations met its minimum criteria (Finanztest 3/2025). - In public health insurance a spouse without own income (up to €565 a month in 2026) and children living in Germany are insured free of charge; in private insurance every person pays a separate premium (§ 10 SGB V). - A German private health insurance policy continues if you move to another EU/EEA country; if you move elsewhere, it ends unless agreed otherwise, and a temporary move can be bridged with a dormant policy (§ 207 (3) VVG, § 15 (3) MB/KK 2009). - From age 55, returning from private to public health insurance is usually ruled out if you were not publicly insured in the previous five years (§ 6 (3a) SGB V). ## Operator - , insurance broker licensed under § 34d (1) GewO, broker register no.
